On behalf of the Physicians
and Staff of Fox Valley Medicine, Ltd., I would like to thank you
for choosing us as your Managed Care Provider.
Fox Valley Medicine, Ltd.(FVM) is an
Independent Physician’s Association located in the Fox
River Valley since 1984. We are the administrative entities
of the managed care network. Our purpose is to review the
services that are requested by HMO members and healthcare
providers to meet guidelines and requirements that are established
by your insurance company. We oversee and coordinate the benefit
inquiry process, claims, eligibility verification, contracting,
utilization management, quality assurance, and credentialing.
As you may know, a major factor
of your managed care plan is the active role of your Primary
Care Physician (PCP). Your PCP will be responsible for directing
all of your healthcare needs. Be sure to contact your PCP
whenever you need to seek healthcare services. Services
rendered outside your PCP's authorization may not be covered
if proper prior authorization has not been obtained.
If you live outside
the FVM service area (service area being, St. Charles,
Batavia, West Chicago, Geneva, North Aurora, Aurora, Naperville,
Plano, Sandwich) we welcome you to FVM. Please note that most
services are provided within our service area, however, not
all services are available at all locations. We have selected
‘Centers of Excellence’ for high profile cases.
These may be within or significantly outside our service area,
such as Chicago and Park Ridge. Our member hospitals (Rush
Copley, Delnor, Provena Mercy, and Valley West) may be utilized
for different diagnoses, to provide the highest level of care
for you and your family.
Please review the section
entitled "Important Steps to Managed Care". This
should assist you in obtaining maximum benefit of your insurance
plan. Please read the handbook given to you by your
employer and be familiar with all of your benefits. This handbook
contains critically important information.
If you add a family member
through birth, marriage or adoption, please remember to enroll
this new member through your employer group. Even if you are
already paying for "Family Coverage", the plan must
have the name and birth date of each new member.
HMOI and Blue Advantage HMO members needing behavioral health services may recieve them through Linden Oaks
Medical Group at Edwards Hospital in Naperville. Please call 1-888-969-7874 between the hours of 9:00 and 4:30 to register.
