If your managed care (HMO/PPO) coverage is the secondary carrier, it is necessary to follow all Fox Valley Medicine procedures (i.e. obtaining primary care physician authorization) in order to assure coverage by the managed care (HMO/PPO) insurance. This is especially important in third party liability cases (i.e. auto accidents, slip and fall cases, etc.) and for married couples who both carry insurance.
