Managed care insurance benefits may vary depending upon the benefit package purchased by your employer. Some managed care insurers have made arrangements with specific physicians and companies for the provisions of certain services (i.e. Routine Eye Care, Psychiatric Services, etc.) and you should be aware of and seek services only from these providers. Referral and/or treatment rendered, but not covered under your plan will be your financial responsibility.
