Should you receive a second bill for the same service, please phone our
Member Service Department at (630) 482-9758.

All claims require review by Fox Valley Medicine, Ltd. prior to payment to verify pre-authorization of the service.

Note: Do not forward bills to your managed care (HMO/PPO) provider, or to your doctor, as that will delay payment of the claim.
