Specific facilities provide laboratory tests, X-rays and other diagnostic imaging. If you need such procedures, your PCP (Primary Care Physician) will direct you to a conveniently located, approved facility.
Please be sure to bring your health plan member ID card, the order from your physician, and your referral (when required) with you to the appointment.
