The provider portal, EZ-Net, is managed by Innovista Health Solutions. Through the portal you can verify eligibility, request and/or view authorizations, and check claims status and payments.
If you are a new user and need access to the portal, please complete the Provider Portal Access Request form and e-mail to the Provider Relations Representative, email@example.com. All new user requests must be in writing.
If you need to reset your password for the portal, e-mail firstname.lastname@example.org. When sending the e-mail request, please indicate in the subject line of the e-mail "Password Reset". The following information must also be included in your e-mail:
• Phone Number
• Practice Name
• Physician Name(s) Associated with Practice.
