The provider portal, EZ-Net, is managed by Innovista Health Solutions. Through the portal you can verify eligibility, request and/or view authorizations, and check claims status and payments.
If you are a new user and need access to the portal, please complete the Provider Portal Access Request form and forward to the Provider Relations Representative. All new user requests must be in writing.
If you need to reset your password for the portal, e-mail firstname.lastname@example.org. When sending the e-mail request, please indicate in the subject line of the e-mail "Password Reset". The following information must also be included in your e-mail:
• Phone Number
• Practice Name
• Physician Name(s) Associated with Practice.
